Current after school members only have to pay for the field trip portion of the day.

Registration ends at 5pm on the day before camp so we can plan staff accordingly.

Pick up and Drop off is in Suite 209.

Open 9am-6pm. Drop off begins at 8:45am. Children must be picked up by 6:05pm or late charges will apply.

AGREEMENT

Read Carefully Before Signing.

In consideration of being permitted to participate in any way in the Martial Arts Program indicated below and/or being permitted to enter for any purpose to any restricted area (here in defined as any area where in admittance to the general public is prohibited). I, being the named participant below, agree:

1. I fully understand and acknowledge that:

  • There are risks and dangers associated with participation in martial arts events and activities which could result in bodily injury partial and/or total disability, exacerbation of existing medical conditions, paralysis and death.
  • The social and economic losses and/or damages, which could result from these risks and dangers described above, could be severe and ongoing.
  • These risks and dangers may be caused by the action, inaction or negligence of the participant or the action, inaction or negligence of others, including, but not limited to, the Releasees named below.
  • I understand that I am not required to perform any activity.  If at any time, I feel that participating in such activity may create injury, I will refrain from participation.
  • There may be other risks not known to us or are not reasonably foreseeable at this time.

2. I accept and assume such risks and responsibility for the losses and/or damages following such injury, disability, paralysis, or death, however caused and whether caused in whole or in part by the negligence of the Releasees named below. 

3. I HEREBY RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE Triumph Fight Academy Inc., the martial arts school, club, instructor/s, other participant/s, club members or the martial arts facility used by the participant, including its owners, managers, promoters, lessees of premises used to conduct the martial arts event or program, premises and event inspectors, underwriters, consultants and others who give recommendations, directions or instructions to engage in risk evaluations or loss control activities regarding the martial arts facility or events held at such facility and each of them, their directors, officers, agents, employees, all for the purpose herein referred to as “Releasee”…From all liability to the undersigned, my personal representatives, assignees, executors, heirs and next of kin for any and all claims, demands, losses or damages and any claims or demands therefore on account of any injury, including but not limited to the death of the participant or damage to property, arising out of or relating to the event(s) alleged to be caused in whole or in part by the negligence of  the Releasee or otherwise. 

4. I HEREBY acknowledge that THE ACTIVITIES OF THE EVENT(S) ARE VERY DANGEROUS and involve the risk of serious injury and/or death and/or property damage.  Each of THE UNDERSIGNED also expressly acknowledges that INJURIES RECEIVED MAY BE COMPOUNDED OR INCREASED BY NEGLIGENT RESCUE OPERATIONS OR PROCEDURES OF THE RELEASEE(S).

5. EACH OF THE UNDERSIGNED further expressly agrees that the foregoing release, waiver, and indemnity agreement is intended to be as broad and inclusive as is permitted by the law of the Province or State in which the event or program is conducted and that if any portion is held invalid, it is agreed that the balance shall, not withstanding, continue in full legal force and effect.

6. On behalf of the participant and individually, the undersigned partner(s) executes this Waiver and Release.  If, despite the release, the participant makes a claim against any of the Releasee(s), the undersigned will reimburse the Releasee(s) for any money which they have paid to the participant, or on his behalf, and hold them harmless.

7. I understand that all activities within the premises are monitored and may be recorded with both video and audio equipment. 

8. Health and Safety Rules - Safety of our members and instructors is the top priority at Triumph Fight Academy, Inc.  While there is always a risk of injury or illness, there are certain actions that every member of the gym has to follow for everyone’s safety and health.  If you have any concerns or questions, please speak to an instructor immediately.  Failure to comply with these rules may result in loss of gym membership.

  • Always come to class clean.  This means clothing is clean and recently washed, oral hygiene is followed and toe and fingernails have been trimmed (no white showing)
  • Any open cuts, scratches or scraps should be covered/bandaged.  If the bandage comes off during class, stop participating until the bandage can be reapplied
  • If you have a preexisting injury prior to class, notify your instructor and decide if modifications should be made to training or halted
  • If you experience pain, injury or loss of function during class, stop your activity and notify your instructor immediately
  • If you are working with a partner, and they express pain, injury or loss of function, either verbally or through body language, stop the activity immediately
  • If your partner physically or verbally “taps”, stop the technique immediately
  • Never “crank” a choke or joint lock technique rapidly.  Instead, apply consistent and increasing pressure giving your partner a chance to “tap” and avoid injury
  • If you notice blood during class, stop and determine the source.  Immediately clean and bandage the laceration and/or stop the bleeding fully before returning to activity.  Sterilize any floors or surfaces that were exposed to the blood before returning to activity
  • If you are ill and have a fever of over 99 degrees, do not come to class until you are fever free for at least 24hrs
  • Always clean your sparring equipment after every use with appropriate sterilization techniques
  • No shoes are allowed on the mats unless preapproved by the instructor
  • After every class and as soon as possible, take a shower/bath and fully clean yourself
  • If you find yourself getting angry or upset, stop what you are doing and take a moment to calm down
  • No sparring unless approved by an instructor and they are present


I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATESET EXTENT ALLOWED BY LAW.
